Fees and Forms
Payment Options
Whether you are looking for weekly sessions or need something that is a little more flexible to fit your lifestyle and budget, I offer multiple options. We can discuss whether self-pay, out-of-network services, or filing with insurance fits best. I accept payment via cash, check, FSA/HSA, and all major debit/credit cards.
Insurance Benefits
I am an in-network provider for Aetna, Blue Cross Blue Shield, and United Healthcare/Optum. For all other insurances, I am an out-of-network provider. Clients may get reimbursed for psychotherapy sessions even when seeing an out-of-network provider.
NOTE: If you have an HMO with BCBS, it is most likely that your mental health benefits will be contracted out to Magellan Health and will need a therapist who accepts that insurance.
Out-Of-Network Benefits
Insurance companies often offer “out-of-network” benefits that can cover psychotherapy services. If you would like to pursue therapy with out-of-network benefits, I can provide you all the documentation necessary to submit claims for reimbursement from your insurance company. Payment for out-of-network sessions are expected in full at the time services are rendered. I am happy to answer any questions you have regarding insurance or out-of-network processes and will assist you with claims if you experience any difficulties.
**Please contact your insurance provider to verify your mental health benefits for “out-of-network” services.
Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give clients who don’t have insurance, or who are not using insurance, an estimate of the expected cost for medical services.
You have the right to receive a Good Faith Estimate (GFE) for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your health care provider, and any other provider you choose, for a GFE before you schedule a service. If you receive a bill that is at least $400 more than your GFE, you can dispute the bill. You will receive a copy of your GFE.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.

New Client Forms
If you are a new client, please take a moment to familiarize yourself with the forms below. Please complete the forms and bring them with you to your initial assessment. If you would like me to coordinate care with another provider (i.e. Primary Care Physician, Psychiatrist, etc.), be sure to complete the “Consent to Release” form. No one enjoys paperwork, but coming prepared for your appointment will ensure we can focus more on therapy and less on administrative matters.
